Membership Cancellation Request

All cancellations must be approved by the center manager and your account must be current.

Cancellation agreement per membership contract: You may cancel your membership with 30-days written notice. Your cancellation will be effective 30 days after the final dues payment has been made. All unused sessions expire 60 days from the final EFT date. All cancellation requests must fill out a Membership Cancellation Request Form. No refunds of unused massage sessions will be given.

I understand the cancellation process. I understand that I will be charged one final payment. My membership pricing will be in effect for 30 days following my final EFT payment. After 60 days following my final EFT, I will forfeit any unused massage.
